HOW TO REGISTER
Register online by following the step-by-step instructions. This time-saving option provides immediate and secure transmission of information and payment. Online registration is open until June 14, 2013, however you may still register on site.
Register by mail by filling out the paper registration form, AM13 registration form including payment information, and mailing to Evangelical Covenant Church, Attn: Annual Meeting registrar, 8303 West Higgins Road, Chicago, Illinois, 60631. All paper forms should be mailed by June 10, 2013, to ensure they reach the events office before the Annual Meeting.
Register by fax by filling out the paper registration form, including payment information (credit card only), and faxing to (773) 596-2494. Registration by fax is open until June 14, 2013.

REGISTRATION FEES INCLUDE
For delegates
- Annual Meeting delegate name badge
- Voting and seating privileges (in accordance with the Constitution and Bylaws of the Evangelical Covenant Church)
- Annual Meeting notebook and business session printed materials (received at the Annual Meeting)
- A copy of the 2013 President’s Report and Covenant financial reports
- Opportunity to attend delegate orientation
- Availability for children’s ministry programs and child care at the designated fee with advance reservation
For general conferees, retired Covenant pastors, NPTS, and CHET students
- Registered attendee name tag
- Seating as an observer at the Annual Meeting
- Copy of the 2013 President’s Report and the Covenant financial reports
- Opportunity to attend delegate orientation
- Availability for children’s ministry programs and child care at the designated fee with advance reservation
CANCELLATION POLICY
Annual Meeting registration may be canceled at any time before June 14, 2013, via mail, fax, or email (cancellations cannot be accepted over the phone). A $50 cancellation fee will be applied. No refunds after June 14, 2013, except in case of emergency.

Substitutions will be accepted from the same church or institution provided the registration and fees are the same for the new registrant. Please mail, fax, or email your substitution request, including the name, address, and telephone number of the substitute.
