Medical Benefits

Administered by Highmark Blue Cross Blue Shield

• PPO plan with across-the-country and around-the-world coverage
• No pre-existing condition issues, even for pregnancy, as long as new enrollees had at least 12 months of health coverage prior to enrolling (children under 19 never have pre-existing condition exclusions)
• Low $300 deductible for individual plans and $600 deductible for family plans
• 80% coverage of most in-network expenses after deductible
• $1,500 maximum out-of-pocket cost per year (after deductible) for individual plans and $3,000 maximum for family plans
• $20 co-pays for office visits, including in-network therapy and counseling

Dental Benefits

Administered by Delta Dental

• 100% coverage of all in-network preventative and diagnostic expenses, including checkups and cleanings every six months, dental x-rays, and fluoride treatments for children under age nineteen
• Increased number of cleanings per year for individuals who have diabetes, are pregnant, have periodontal disease, have kidney failure, have high-risk cardiac conditions or suppressed immune systems, or are undergoing cancer-related radiation or chemotherapy (download the Enhanced Benefits Program below for more details)
• $25 deductible ($75 maximum deductible per family)
• 80% coverage of basic dental work, including fillings, oral surgery, endodontics, and anesthesia
• 50% coverage of more extensive work such as implants, crowns, bridges, and dentures
• 50% coverage of orthodontia (lifetime maximum of $1,500 per person)
• $1,500 maximum coverage per year, per person which can be carried over to the next calendar year for a maximum of $3,000 per year, as long as you were enrolled with Bethany Benefit Service for the entire previous year and filed at least one claim the previous year

Vision Benefits

Administered by TruAssure

• Many in-network, large eye care providers (Access Network)
• $20 co-pay for in-network, yearly exams
• $100 allowance, and 20% off balance over $100 for glasses frames every 24 months
• $20 co-pay for standard, plastic glasses lenses or $80 allowance, then 15% off balance over $80 for contacts every 12 months
• Discounts for Lasik/PRK procedures

Life Insurance

• $100,000 in coverage until the age of sixty-five
• Double indemnity in the case of accidental death until the age of seventy
• Benefits for accidental disability or dismemberment
• Available only for full-time, actively working employees

Long-term Disability Insurance

Administered by Unum

• The lesser of 60% of monthly earnings or 70% of monthly earnings less other income is provided in the event of long-term physical or mental disability
• Life insurance provided while on disability
• Health insurance provided for up to twenty-four months by Bethany Benefit Service while on disability (if applicable)
• Pension payments made to the Covenant Pension Fund by Bethany Benefit Service while on disability (if applicable)
• 90-day elimination period
• 24-month maximum benefit for mental illness, alcoholism and drug abuse
• Available only for full-time, actively working employees

Counseling and Therapy Options

Administered by Highmark Blue Cross Blue Shield and Unum

We understand that pastors, missionaries, and other church workers face a number of challenges in their work and home lives. Bethany Benefit Service includes two benefits for counseling and therapy.

Highmark Blue Cross Blue Shield covers individual, family and marriage counseling with an in-network, licensed professional for a low $20 co-pay per visit. Out-of-network counseling and therapy visits can be reimbursed at 60% after the out-of-network deductible has been met.

The Employee Assistance Program offers a variety of options for work-life balance including online resources and toll-free confidential counseling and assistance from master’s level consultants. Call (800) 854-1446 (TDD/TTY: (800) 999-3004 or Spanish: (800) 317-7264) for help with:

International Travel Benefits

Administered by Highmark Blue Cross Blue Shield and Unum

BlueCard Worldwide Medical Assistance offers several services for those traveling and living outside of the United States, including making appointments for you and referring you to doctors. This benefit is included for individuals enrolled in health insurance through Bethany Benefit Service. Emergency Travel Assistance through Unum is included for all full-time employees receiving life and long-term disability insurance.
