To file a claim in the United States:

If you visit a doctor outside the network or before your enrollment has been processed, you may need to file your own claim to receive reimbursement.  To file a claim:

  • Complete the appropriate claim form, found on our resources page.
  • Make a copy of the completed form and send it, along with any additional information available (ie copies of bills, doctor's notes, etc.), to the address listed on the claim form.
  • Allow 3-4 weeks for processing.  If no response is given, contact the appropriate insurance company to find out the status of the claim (see our contacts page).
  • If you are unable to resolve the claim with the insurance company yourself, Bethany Benefit Service will act as your advocate to ensure that eligible claims are paid in a timely manner.  Contact Phil Gonzalez, our claims specialist, at (773)907-3363.

To file a claim from outside the United States:

When overseas, in-network doctors are very scarce.  When receiving medical attention or filling prescriptions, you will most likely be required to pay the balance up front and request reimbursement by filing a claim after the date of service.  Send claims as soon as possible after the date of service to avoid delays and complications in the approval/reimbursement process.

Information to provide for each claim:

  • Claim form.  Please fill out the appropriate claim form completely (found on our resources page).  Include the primary cardholder's name, the patient's name, your ID number, and our group number (found on your card).
  • Details of what happened on the date of service (not needed for Express Scripts)
  • Your bill or a letter from the doctor explaing the appointment with a detailed description of the amount owed
  • An English translation of your bill and/or letter from your doctor
  • The exchange rate to American dollars
  • Be sure to write dates in this format: Month/Day/Year

How to file a claim:

  • Copy all information before sending in the mail.
  • Send all information (via fax or mail) to Bethany Benefit Service, not directly to the insurance company.  This saves time later if there are any complications in approving the claim.
  • All claims should be processed and approved within 6 weeks of their being received.  If you are a missionary working with the World Mission Department, a reimbursement check will be sent to the World Mission Department and deposited into your personal account once the claim has been approved (unless alternate arrangements have been made).
  • Contact Bethany Benefit Service for updates on the status of your claim.  If you do not receive a response or reimbursement within 6 weeks of sending your claims.
