Criminal Record Screening Form

A criminal record screening must be completed by everyone seeking credentials with the Covenant Church. It is also required for those who wish to be on the denomination's Open to Call List. There is a $30 cost for this screening. Please download the form, print it out, complete it, and return it to the Department of the Ordered Ministry with a check for $30 made payable to the Evangelical Covenant Church.
