Criminal Record Screening Forms

A criminal record screening background check must be completed by everyone seeking credentials in the Covenant Church. It is also required for those who wish to be on the denomination’s Open to Call List. Please download and print these forms, and mail them to the Department of the Ordered Ministry with a check for $75 payable to the Evangelical Covenant Church. Or, you may pay with a credit card by using the credit card payment form (below).
